Account Builder Enclose this form with additional investments to ensure the proper processing of your investment.
Account Change Request Update addresses, contribution methods, bank information and successor account owner information.
Certification of Authorized Individuals Name the individuals authorized to sign on behalf of your corporation, unincorporated association, or partnership.
Change of Beneficiary Change the Designated Beneficiary on your account.
Designation of Successor Custodian for UGMA/UTMA Accounts Complete this form to designate a successor custodian for your UGMA/UTMA EdVest account.
Distribution Request Complete this form to request a distribution from your account.
Investment Change Form Change your investment option. Investment changes for an existing account are permitted up to the annual investment change limit without a change in Designated Beneficiary.
Name Change Authorization Change your name or the name of the Designated Beneficiary on your EdVest account.
Payroll Direct Deposit Set up automatic deposits of all or part of your paycheck to your account.
Power of Attorney Establish a Power of Attorney (POA) on your account. A POA may not be used for a 529 Plan Program UGMA/UTMA custodial or guardianship account.
Rights of Accumulation and Letter of Intent Establish Rights of Accumulation (ROA) and Letter of Intent (LOI) on your EdVest account.
Rollover From a Qualified Tuition Program Transfer funds from another state's Qualified Tuition Program into your EdVest account.
Systematic Exchange Plan Complete this form to establish a Systematic Exchange Plan between two EdVest accounts.
Systematic Withdrawal Plan Complete this form to establish a Systematic Withdrawal Plan on your EdVest account.
Transfer of Ownership Complete this form and an Account Application signed by the new Account Owner to transfer ownership of your EdVest account(s) to a new Account Owner.
Investment Certificates
If you will be making a contribution as a gift, you may print a certificate and present it to the student or the account owner.
